Obstructive Sleep Apnea and Ischemic Stroke ZHUZHICHENG
Glossary l Ischemic Stroke l Obstructive Sleep Apnea(OSA) l Obstructive Apnea–Hypopnea Index (OAHI) l Polysomnography l Prospective cohort study l Cox proportional hazards model
Obstructive Sleep Apnea Symptoms Definition • Pause in breathing • 10 seconds/hour • Airway obstruction Correlation • High blood pressure • Heart disease • Diabetes • • • Loud snoring Unrefreshed sleep Excessive daytime sleepiness Gasping or choking from sleep Mood change Cognitive dysfunction
Methods—Prospective cohort study l Sample: 5422 (2462 Men + 2960 Women), 8. 7 years l Measurement: Ø Baseline examination: OSA: Apnea–Hypopnea Index(OAHI) Polysomnography Other covariates: BMI, BP, age, race, smoking status, medication, diabetes status, alcohol use, cholesterol, atrial fibrillation Ø First occurrence of stroke: mailing & surveying l Statistical analysis: Cox proportional hazards model
Results
Conclusion There is a significant positive association between ischemic stroke and obstructive sleep apnea in men.
Discussion l Man VS Women: Ø Stroke risk factors, incidence rates, and outcomes differ in men and women. Ø The prevalence of OSA increases more steeply with advancing age in women than men. l Mechanism : Ø Cerebral tissue oxygenation Ø Blood flow regulation Ø Atherogenesis
