Provide ICD-10 Diagnosis codes (not ICD-9) • You will decide upon a primary (presumptive) diagnosis, which should appear as the first diagnosis on the list of 3 differential diagnoses on the final SOAP note. • You will construct the plan of care based upon the primary diagnosis. • You need to provide pertinent positive and negative findings from the subjective history and objective physical examination to support each diagnosis 20 © 2013 Chamberlain College of Nursing, LLC.
