Requirements • Final Forms You must register your daughter online with Final Forms on the NDA website. • KHSAA Physical: Your daughter’s 6 page KHSAA physical should be uploaded to Final Forms before the first tryout day or first practice. • St. E’s Trainer Form: St. E’s has a release form that must be completed and turned into the Athletic Trainer. • Participation Fee: This fee will be billed through FACTS. This will happen in September for fall sports, November for winter sports and March for Spring sports. • Fundraisers: Coupon Books & Golf Outing – Coupon Book selection must be made by August 27 th in Final Forms. If an option is not selected your athlete will owe the full fundraising fee and will not have books to sell.

This certifies that the student athlete is physically fit to participate without undue risk and includes insurance information and acknowledgment of risk. • It is required for each student athlete to have medical insurance. • NDA does not carry secondary medical insurance for the student athletes. • St. Elizabeth Sports Medicine provides a full time athletic trainer for all games and practices. • St. E’s Trainer Form must be filled out and on file in order to see the trainer. • NDA Trainer: Melissa Fields • melissa. fields@stelizabeth. com

Eligibility Guidelines • Signed permission forms for all athletic related school absences are required.

Eligibility Guidelines • Signed permission forms for all athletic related school absences are required. • An athlete failing 2 or more classes will be suspended from play and practice for two weeks. • A student athlete on disciplinary probation may not participate until the probationary period has been satisfied. This is determined on a case by case basis. • Student athletes must be present for at least 4 consecutive classes to be able to participate in athletics. • Any practices or athletic contests missed due to detention will be handled at the discretion of the individual coach.

Play Like a Champion Today • Character and Faith Development through Athletics • Coaches have been through this training. A parent training is now required by the Diocese. NDA will offer one in October. • From their website: “Play Like a Champion sees coaching as a ministry. Coaches’ primary role is to serve children to fully develop their human potential as God’s daughters and sons. The ministry of coaching involves protecting the joyful dimension of sports as competitive play and the educational dimension of sports as fostering children’s physical, social, moral, and spiritual development. ”
