Current OHCW case management agencies are Care. Star, Care. Source, and Council on Aging • During the NF stay, provide updates to the waiver Case Manager about changes in expected length (permanent, long-term, proposed discharge date, etc. ) • Bill under MITS waiver revenue code while enrolled on fee-forservice Medicaid waiver 1

Care Ohio Plan (MCOP) • Send an ODM 9401 per instructions on 9401 form • Contact the MCP/MCOP for prior authorization and/or level of care process, and inform MCP/MCOP of expected length of stay (permanent, short-term, long-term, etc. ) • During the NF stay, provide updates to the MCP/MCOP about changes in expected length (permanent, long-term, proposed discharge date, etc. ) 2

NF admissions on Specialized Recovery Services Program (SRSP) • Send an ODM 9401 per instructions on 9401 form • Contact the SRS Recovery Manager regarding admission status and expected length of stay (permanent, short-term, longterm, etc. ) • During the NF stay, provide updates to the SRS Recovery Manager about changes in expected length (permanent, longterm, proposed discharge date, etc. ) • If permanent NF stay or long-term (>90 days), contact SRS Recovery Manager to request SRSP disenrollment • Disenrollment will be entered into Ohio Benefits by SRS Recovery Manager; CDJFS must run the case in Ohio Benefits in order to close the SRSP span in MITS 4
