Submit photocopies of certificated marriage and/or birth certificates (or adoption records) Failure to submit required documents will result in loss of coverage for spouse and/or child(ren). HEALTH CARE SAVINGS ACCOUNT Health Savings Account (HSA) maximum is $3, 600 for single coverage and $7, 200 for family coverage (if you are 55 years old or older, you may contribute an additional $1, 000). If you have Medicare Part A, you are not eligible to contribute to an HSA. Cone will still provide seed money for employees in the SAVE Plan (unless you earn $130, 000 or more per year). Limited Flexible Spending Account. Can only be used for dental and vision expenses until your medical plan deductible is met. Maximum allowed contribution to Limited Flexible Spending Account is $2750.
