Durvalumab (Imfinzi) FDA Indication -NSCLC, unresectable. -Urothelial carcinoma, locally advanced or metastatic. Target/ MOA -Durvalumab is a human Ig G 1 kappa monoclonal antibody which blocks programmed cell death ligand 1 (PD-L 1) binding to PD-1 and CD 80 (B 7. 1); PD-L 1 blockade leads to increased T-cell activation, allowing T-cells to kill tumor cells. Administration - 10 mg/kg IV infusion over 60 min once every 2 weeks through an IV line containing a sterile, low-protein binding 0. 2 or 0. 22 micron in-line filter. - Continue dose until disease progression or unacceptable toxicity or a maximum of 12 months. BWW/Precautions -Immune-mediated pneumonitis, hepatitis, colitis, endocrinopathies (thyroid disorders, adrenal insufficiency, T 1 DM and hypophysitis/hypopituitarism), nephritis, and dermatologic reactions (SJS/TEN). Unique ADRs -Fatigue, infection (UTIs), hyperglycemia, hypocalcemia, hyponatremia, hyperkalemia, dermatitis, constipation, increased LFTs and cough.
