NEVADA STATE ONLINE APPLICATION PORTAL (SOAP) PRESENTATION How to complete an Emergency Medical Services (EMS) application using the SOAP System.

Step 1: Access your account Ø Use the following URL to access the SOAP

Step 1: Access your account Ø Use the following URL to access the SOAP log in screen. � https: //nevada. imagetrendlicense. com/portal#/login Ø When logging into your SOAP account for the first time, please read the following instructions. § § OPTION 1: CLAIMING YOUR ACCOUNT: If you currently hold a State of Nevada EMS license, but you have not yet claimed your account in our new on-line system, select "Forgot Username" below and follow the instructions. OPTION 2: CREATE AN ACCOUNT: If you do not have a State of Nevada EMS license, you can create an account by selecting the "Create Account" button below. ØIf you have any issues accessing or logging into your SOAP account, contact the Nevada State Emergency Medical Systems Program (SEMSP) at Health. EMS@health. nv. gov and we will be happy to assist you.

Step 2: Starting an Application continued… � The SOAP system will only allow a provider to have one active application at a time. If you need to continue an application you already started, you can do so by accessing your provider dashboard and hitting the “Form pending completion” link.

Add any EMS endorsements you currently hold and want to renew.

Step 3: Completing the application Certification level and continuing education: Part 2 � � Every EMS provider must upload their continuing education and certification cards to the supplemental training section, Page 2. To add training, select the “Add training” button and list the following: Name of the Course Date the course was completed Location Upload the certificate of completion for that course � Add the topic hours, as appropriate � Once you upload your continuing education into the supplemental training section, “Save and Continue” to proceed. � �

If you’re not licensed, but you were directed to the attendant license page, please revisit the demographics, Page 1, and ensure you selected the appropriate response in the fee calculation field. Licensed Providers Upload a copy of your driver's license � Your signed physician statement � Answer the convictions questions � Select your primary service � Select if your agency pays your renewal fees � Once complete, “Save and Continue” to proceed. �

Step 5: How to check your application status � Providers can view their current application status by logging into your SOAP account, select the “Applications” tab, select “Continue. ” This view will show you every application a provider has completed and submitted to the Nevada EMS office.
