Host Home Program Neuro. Restorative’s innovative Host Home Program provides participants with the opportunity to transition from facility-based services to life in the community. Participants will live in the home of a primary support person known as a Mentor. We carefully match participants with Mentors and strive to create meaningful relationships that lead to lasting success for program participants and the Mentors they live with.

Service Provision • • Continuous evaluation of discharge plan Community Integration Family Training & Education Social & Recreational Activities Vocational Rehabilitation Medication, medical and disability management Overall coordination of community resources and supports Intended outcome: evaluation of independent living.
