How we support clients and carers • Information, Advice and Support Service in Hertfordshire

How we support clients and carers • Information, Advice and Support Service in Hertfordshire is commissioned by HCC, 4 part time posts of 25 hours per week. 2 Watford, 2 Stevenage: Judith Smyth and Jacqui Smith • Home visits, telephone or email. The coordinators offer specialist stroke information tailored to stroke survivor or carer’s needs, goal setting, emotional support, factsheets, signposting, referral to other support services and advocacy as required. • The service is offered to anyone affected by stroke to include stroke survivors, family, carers and friends. • Referrals primarily come from the Stroke Specialist Nurses and therapists on the stroke unit at Lister and following rehabilitation at Danesbury Stroke Helpline 0303 3033 100 stroke. org. uk
