NCLEX application You register online to take NCLEX on the Pearson Vue website: http: //www. pearsonvue. com/nclex/ Cost $200. 00 Please see the 2020 NCLEX Examination Candidate Bulletin. https: //www. pearsonvue. com/nclex/ You must indicate the state in which you are registering and may only select 1 state for Licensure by Examination. • Be sure to spell your name in identical fashion on all forms. • NCLEX Program Code: • • Avery Point: US 69500200 Stamford: US 69500300 Storrs: US 69500100 Waterbury: US 69500400 • The program code identifies you as a UConn nursing graduate from the CEIN/BS program. • Use mailing and email addresses where you can be reached after graduation. • You can physically take NCLEX in the state where you live but apply for licensure in another state. You indicate on the NCLEX application where you are licensing and Pearson Vue will send your results to the correct state board.

Application asks for the month and year of graduation. The application will not accept a future date. • Application is valid for one year. • Submit no later than graduation.

Each state has different requirements. • Please provide Pam Cartledge with documentation on what the state board needs from the School of Nursing, including a web link or email forms, if applicable to Pamela. Cartledge@uconn. edu. • Degree Conferral date is December 20, 2020.

Requesting an official transcript • • Registrar’s information webpage for transcripts: https: //registrar. uconn. edu/transcripts/. Request an official transcript through Student Admin after your degree is posted. Degrees are not posted until after graduation when Degree Audit has confirmed all grades. Cost: • e-transcript: $5. 00 • Paper transcript: $9. 50 (includes paper transcript plus postage in US) • Check with the state board where you are licensing if an e-transcript is accepted. • Check that your name and address are correct in Student Admin. Diplomas are mailed to the address in Student Admin. Your name is printed as it appears in Student Admin on your diploma and transcripts.

Authorization to Test (ATT) • Once your application is complete, the State Board of Nursing reviews your application and notifies Pearson Vue that you approved to take NCLEX. • Pearson Vue emails an authorization to test (ATT) to you. • Be sure you have access to email to receive the ATT. • Go to www. pearsonvue. com/nclex to sign up for a date to take NCLEX. • You must take the NCLEX exam prior to the expiration date on the ATT. Average time limit is 90 days but varies by state. After that the ATT is no longer valid. • Bring the ATT with you on the day you take NCLEX. • The name on your ID must match exactly the name on your ATT or you will not be allowed to take the NCLEX exam. • You do not need to take the NCLEX exam in the same state where you are licensing so long as you have indicated the state on the NCLEX application.

Potential holds on transcripts • Be sure to check your Student Admin account for any potential holds that could prevent a transcript from being printed. • Most common – Bursar holds • Other holds: unpaid parking tickets, financial aid exit holds • Financial Aid Exit hold or Perkins Loan Exit hold – contact Financial Aid office or check the website, https: //financialaid. uconn. edu.

AACN Exit Assessment survey • The faculty review your input on the nursing program when considering future program changes. • Survey information will be sent via CEIN 20 listserv.

Alumni Form • Please complete the Alumni form sent by email. • Return to pamela. cartledge@uconn. edu by December 14 th. • The School of Nursing and the Alumni Association contact all alumni about school events and email an annual publication from the School of Nursing.
