Completing the Referral Form 1. Select the required patient in EMIS and click ‘Add’ -> ‘Document’ -> ‘Create Letter’ -> open the Telederm Referral Form 3. Complete the referral form • Consent boxes • Description of current problem • Do not insert the images 2. The Telederm Referral Form will then be loaded (ensure the Telederm Referral Form loads in ‘Print Layout’). 4. To save in the patient’s medical record, click ‘File’ -> ‘Save and Close’ 2

Uploading Images to Patient Record 1. Open a blank Word document. 2. Ensure the document is in ‘Print Layout’. If you want you can use the image template form on the GP website (see below) 3. Within the document: • Select where you want to insert the image • click ‘Insert’ -> ‘Pictures’ 4. In the Navigation Window, navigate to the i. Phone containing the image and double click on ‘Internal Storage’ 4
5. Double click on the ‘DCIM’ folder to open 6. A list of folders containing images will now appear, double click on the folder containing the image – in the below example ‘ 100 APPLE’ 7. A list of all images will now appear, double click the image you wish to upload to the patient record 8. Clearly label the images so that the consultant will know which part of the body the rash/lesion is on. For example: • • Lesion Lower left leg - Front 5
9. Where images have been taken of multiple rashes/lesions it is best practice to ensure that images are labelled. For example: • • 10. Once all images have been inserted and labelled, click once on one of the images Lesion A Lesion B 11. Once all images have been inserted and labelled, click once on one of the images 12. Compress the images 6
13. Ensure that you select the option to compress all images by either: • Unselect ‘Apply only to this picture or • Select ‘Apply to images in this document This depends on the version of Word you are using. 14. Once all images have been inserted and labelled, save the document in a shared folder 15. In the Patient Record navigate to the documents module 7
16. Once in the ‘Documents’ module, to begin adding an attachment, press on the ‘Add’ Button and select ‘Attach Document’ 17. In the Navigation Window, navigate to the file where the image document is saved and upload the form 8
18. Once you have inputted the document click on the magnifying glass and search for: • Referral to Teledermatology Service 19. To complete the upload click ‘File’ at the bottom of the screen. 9
