NAME: AMIRUL AMIN BIN AHMAD NAZRI MATRIC NO: MKK 152003 CASE TITLE: KIELLAND PLATFORM CAPSIZE ACCIDENT
Case Name • Capsize of Offshore Oil Drilling Platform • At around 6. 30 pm March 27, 1980 Date Place Scale of Accident • The North Sea • Located approximately 320 km east of Dundee, Scotland • 123 workers among the 212 people on board were killed in the accident
What Happen?
Fatigue crack growth in brace D 6 initiating from pre-existing cracks in the fillet welds between a hydrophone support and the brace The five other tubular bracings connecting to the vertical column D broke off due to overload The column D became separated from the platform The platform became unbalanced and capsized
Technical causes & consequences Human and organizational factors Fatigue failure of one brace (initiated by a gross fabrication defect) Fabrication defect due to bad welding, inadequate inspection Ultimate progressive failure of braces No fatigue design check carried out Progressive flooding Codes did not require structural robustness (damage – tolerance) Inadequate evacuation (e. g. lifeboats) and rescue operation Damage stability rules did not cover loss of a column Failure to shut doors, ventilators etc. contributed to the rapid flooding and capsizing Evacuation not planned for an accident of this kind Lack of life boats, survival suits Long mobilizing time for rescue vessels/helicopters
