Repeat Clarifications Ø DOH verses OPWDD consent & enrollment forms (both acceptable). New versions up on Website Ø Can I use prefilled consent forms? 6 Ø You MUST have both the HHCM AND the Basic HCBS Plan Support enrollment forms with you. Do not assume you know what they will choose. Ø You must have a blank form option. Ø Where do I send the Consent/Enrollment Forms? Ø If Care Design NY, keep on site.
