Trails System by aligning activities with Healthier Somerset CHIP objectives, partnering with the Montgomery Twp. Planning Dept. and participating in the planning and implementation of Montgomery Trails Day. KEY ACTIVITIES § Solicited input from Montgomery Twp. Planning Dept. and Somerset County Planning Dept. to develop implementation action items for Walk, Hike, Bike Plan § Partner with Montgomery Twp. Residents’ Rates of Illness Somerset County Disparities White age-adjusted mortality rate (2007 -2016) for heart disease was more than double that for Blacks. Rate of low birth rate babies is, and has been, higher among Blacks than for both White and Hispanic residents since 2011. The percent of Somerset County residents reporting high cholesterol is more than double the Healthy People 2020 Target. The general hospital inpatient use rate in Somerset County (137. 7 per 100, 000) is lower than that of the State, but the zip codes of Kingston, Manville, Somerset, Raritan, and South Bound Brook all have hospital use rates well above the statewide rate.

Residents’ Rates of Illness Mercer County Disparities In 2016, the mortality rate for heart disease deaths among Whites was more than 6 times the rate of Hispanics. The rate of low birth rate babies was, and has been, higher among Blacks than for both White or Hispanic residents since 2011. The low birth rate among Blacks was nearly double the rate among Whites. The percent of Mercer County individuals reporting high cholesterol (43. 1%) is more than triple the Healthy People 2020 target of 13. 5%. The inpatient use rate in Mercer County (171. 7 per 100, 000) was higher than that of the State, and the zip codes of Trenton/Ewing zip code 08618, Trenton zip code 08628 and Trenton/Hamilton (08609) all have hospital use rates well above the statewide rate.

Key Mental Health & Substance Use Concerns Somerset County 12. 5% of Somerset County residents reported a history of depression, up from 9. 2% in 2012. The percent of residents reporting heavy drinking was higher in Somerset County compared to Hunterdon and Morris. Between 2012 and 2016, there was an increase in the rate of ED visits for substance abuse in all Somerset County age cohorts, except those aged 0 -17.

Key Mental Health & Substance Use Concerns Mercer County In 2016, 14. 9% of Mercer County residents reported a history of depression, down from 15. 4% in 2014. The percent of Mercer County residents reporting heavy drinking was higher than the state and Camden and Middlesex County. In 2016, Mercer County had a higher rate of residents with an ED visits for substance abuse among all cohorts except for those 0 -17 as compared to the state.

Somerset County Chronic Disease Statistics 4 of the top 5 leading causes of death for Somerset County are chronic diseases: this includes cardiovascular disease, cancer, stroke, and Alzheimer’s. Diabetes • Diabetes was a top concern among Hispanics and those with lower incomes. • Nearly 10% of Somerset County residents reported diabetes, higher than NJ residents’ statewide rate and rates for residents of Hunterdon and Mercer Counties. Cancer • Patients using a hospital service from Skillman reported the highest rate for cancer diagnosis (45. 62/1, 000). • Somerset County male survey respondents and those over 65 were the most concerned about cancer. Cardiovascular Disease Other Chronic Disease • 26. 7% of Somerset County residents indicated that they were told they had high blood pressure. • Chronic heart failure was of highest concern to those over 65. • A third of Somerset County residents reported high cholesterol. • The average annual mortality rate for Alzheimer’s Disease increased 72% between 2007 and 2016. • The percent of Somerset County residents reporting arthritis increased from 18. 8% in 2013, to 22. 5% in 2016. • Obesity was the number 1 concern among survey respondents in Somerset County and the 2 nd highest priority health issue reported from the Workplace Wellness focus groups.

Mercer County Chronic Disease Statistics 4 of the top 5 leading causes of death for Mercer County are chronic diseases: this includes cardiovascular disease, cancer, stroke, and Alzheimer’s. Diabetes • Diabetes was a top concern among males, older populations, and those with lower incomes. • The percent of Mercer County residents reporting diabetes increased from 8. 3% in 2011 to 12. 2% in 2016. Cancer • The 2016 rates of residents using a hospital service that had a history of cancer diagnosis was higher in Mercer County (18. 06/1, 000) than in the state (13. 90/1, 000). • Mercer County male survey respondents and those over 65 were the most concerned about cancer. • Mortality rates for cancer in Mercer County have been increasing since 2007. The mortality rate for malignant neoplasm deaths among Black Mercer County residents is higher than the rate statewide among Black residents. Cardiovascular Disease Other Chronic Disease • In 2015, 29. 7% of Mercer County survey respondents reported being told they had high blood pressure. • Aging (47%) was of highest concern to those over 65. • More than one-half of respondents claim to have high blood pressure, high cholesterol and/or a weight problem. • Between 2008 and 2016, age-adjusted mortality rates for Alzheimer’s disease declined by 3. 2%. • Since 2013, the rate for adults with arthritis increased from 22. 1% to 22. 9% and has remained slightly lower than the rate statewide. • Obesity was the number one concern among survey respondents from Mercer.
