Phyzit enables health care providers to offer better health outcomes through reduced hospital readmissions by applying TCM. Phyzit tracks the metrics of TCM so providers can concentrate on patients. Transitional Care Management is key in providing better health outcomes for patients, but managing TCM doesn’t match normal clinic workflow. Phyzit wanted to design a TCM solution, but it needed a secure platform that could reliably handle sensitive information safely while still complying with HIPAA standards. § SOLUTION Phyzit worked with experts from the Microsoft Azure Cloud DX team to build a scalable, redundant, and robust solution leveraging multiple Paa. S and Iaa. S features, including Azure Virtual Machines, Azure SQL Database, Azure Storage, Load Balancer and Transparent Data Encryption. Combining health care expertise and the Azure toolset allows Phyzit TCM to provide a HIPAAconformant Saa. S solution that is secure (encrypted at-rest and intransit), reliable, and available across the United States. § BENEFITS Azure SQL’s Transparent Data Encryption enables at-rest encryption for database records on the fly, with minimal overhead. Azure’s geo-replication and geo-redundancy provide multiple points for disaster recovery, business continuity, and data integrity.
