What is migraine? • Chronic disorder • Recurrent attacks of headache • Complex neurobiological disorder • Core features: Headache, nausea, light and sound sensitivity etc. • Migraine predisposition varies individually
Phases of migraine
Pathophysiology of migraine • Several theories trying to explain the etiology of migraine • The most accepted ones are the vascular and neurovascular theories
Neurovascular theory Cortical depression spreding (CDS) - Reduction in brain electrical activity and decrease blood flow - Release of K+ and H+ activates sensory fibers - Activation of trigeminal & brain stem neurons - Precipitation of vasodilation
Genetic component • Strong familial influence proved in twinstudies • Mutations in voltage gated calcium channels seen in many families, predisposing to migraine • Related to serotonin release in midbrain
Migraine and neuropsychiatric disorders • Comorbid disorders, such as depression, anxiety, panic disorder and bipolar disorder. • There are several researches and theories explaining the associations.
Migraineurs VS non-migraineurs
Treatment • Pharmacological and non-pharmacological approaches • Pharmalogical: NSAIDs (Aspirin, Ketoprofen), Erogot alkaloids, Triptans • Non-pharmacological: Cold packs to the head, Riboflavin and Magnesium • Medication of prevention: - 1 st line: Propanolol, Amitriptylin - 2 nd line: Valproat, Pisotifen
