Folate Trap Hypothesis l l l Cobalamin deficiency and folate deficiency produce indistinguishable hematologic abnormalities The hematologic abnormalities seen in cobalamin deficiency can be completely reversed by pharmacologic amounts of folic acid. The hematologic abnormalities caused by folate deficiency respond only slightly, if at all to large amounts of cobalamin.

Vitamin B 12 Deficiency: Tx l l The traditional approach to treating cobalamin is intramuscular injection of cyanocobalamin. Alternatives include oral tablets and nasal gel. l l l Patients with pernicious anemia will absorb 1 to 2 percent of an oral dose even without IF. One thousand to two thousand micrograms daily of oral B 12 is an alternative to injections. The nasal gel is currently labeled for maintenance use.
Folate Deficiency: Tx l l l Folic acid 1 mg daily will replenish body stores in about three weeks. Patients with malabsorption may require doses up to 5 mg/day for replenishment. Because folic acid, even through food supplementation, partially corrects the hematologic abnormalities of B 12 deficiency but not the associated neurologic deterioration, concomitant B 12 deficiency should be ruled out before starting folic acid therapy.
