Athletic Training Services • Available for: Injury evaluation, management and rehabilitation. • Available to assist with facilitation of sports medicine physician, physical therapy, and concussion appointments at any Cleveland Clinic Facility. • On site at home athletic contests. - Coverage varies depending on schedule of events.
Availability: • Athletic Training Room: Room H 6 Hours: Winter 2: 45 -6: 15 - These hours will vary depending on in season sport practices/games.

West Side Appointment Locations • Many locations available on the west side! - Express care in North Olmsted on Lorain Rd. Physical Therapy locations in Fairview, Westlake, and Avon Sports Medicine physician appointments at various locations. • Walk in clinics available on Saturdays at Cleveland Clinic Sports Health during the fall season. • Concussion appointments Mondays at Sports Health and various locations throughout the week.
Concussion Management • Mandatory baseline testing for contact sports, utilizing Im. PACT and/or C 3 concussion management system • Systematic management and gradual return to play after physician clearance. This is Ohio law! • All concussions must be reported to the athletic trainer • Work with school administration to provide academic accommodations, if required. • Electronic injury tracking • All suspected head injuries must be cleared by a physician (MD or DO). -THIS IS OHIO LAW Ø We attempt to provide the safest, most comprehensive and most up to date care for your athlete.

Additional OHSAA eligibility information: v. Summer school courses cannot be used. v. A students overall GPA has no impact on eligibility. v. High school student – turns 20 – immediately ineligible v. Athlete will be ruled ineligible if they compete on a non- high school team during their season. Eligible to practice and compete ØStudents must be in school by 11: 30 AM to practice or compete in any extracurricular activities. ØStudents must be in school the 2 nd half of the school day. ØIf a student leaves school ill and unexcused they will be ruled ineligible to compete on that school day. ØStudents absent but excused are permitted to practice and compete (note or proof must be provided).

Student and Parent Code of Conduct OHSAA & NOHS Ejection Policy Any student or parent who is ejected or disqualified shall be ineligible for the remainder of that day. In addition, the individual shall be unable to attend and/or participate in two additional contests (football one).

College Bound Athletes Must register for the NCAA clearing house at www. ncaaclearinghouse. net The Clearinghouse processes academic qualifications for all prospective Division I and Division II athletes and determines whether they are eligible to compete.
Make the right choice The NCAA has a staggering statistic : They say that 26% of all incoming scholarship freshman athletes coming into college leave school before the end of their first semester! That’s 1 in every 4 college scholarship freshman. This isn’t a 4 year decision for your child. It’s a 40 year decision that will shape and mold their future.
