• Claim of the amount is varied according to the hospitals, treatment, medicine and illness. • Basic range of coverage : Medical items that are covered by insurance are covered 90% Medical items that are not covered by insurance are covered 80% * Hospitalization period more than one day and Max. 180 days within the insured period

• The claim of amount based on the range or location of the hospital. • There are three levels of hospitals based on range or location : 1. Range : Clinics, Hospitals & Multi Specialty Hospitals* 2. Location : Rural, Urban & Metropolitan City 3. Amount : 10, 000, 15, 000 & 20, 000 * It is not easy to name them or segregate them, but you are entitled to use them as per your need and urgency.

• Required Documents for Medical Insurance Claim Description Required Documents Below 30, 000 ~100, 000 Above 100, 000 - Medical Expenses Statement (Doctor’s Fee & Medicine) - Doctor’s Prescription (Details about disease) If there is no the disease classification Additional Documents code on prescription, Additional Documents are required for the explanation of disease. - Doctor’s Certificate - Details of Medical Fee - Medical Test Report - Treatment reports
