Welcome & Introductions • DHS, DMVA, LASD, CEO, DPSS Purposes of this Input Session: • Provide an overview of the CBEST Program • Provide an overview of the Measure H program elements • Receive community input and feedback about the CBEST Measure H program additions
L. A. County Strategies to Combat Homelessness • In February 2016, the Los Angeles County Board of Supervisors approved strategies C 4, C 5, and C 6 • The RFP process, including all three strategies, was completed in March 2017 • There were 7 successful proposers (community based agencies) serving all 8 SPAs
What is CBEST? History of Benefits Advocacy in Los Angeles • HOPE, BEST, SOAR, DPSS GR to SSI (SSIMAP) County Homeless Initiative: • C 4, C 5, C 6 New Program design is a holistic approach • Benefits advocacy and linkage to housing and services • “Whatever it takes” approach

Original C 4, C 5, C 6 Goals • Screen and serve 10, 000 people per year • Complete 5, 000 SSI/SSDI/CAPI applications per year • Complete 600 Veterans benefits applications per year
Measure H Expansion Our goals have DOUBLED! • Screen and serve 20, 000 people per year • Complete 10, 000 SSI/SSDI/CAPI applications per year • Complete 1, 200 Veterans benefits applications per year
Who is eligible for CBEST? Individuals with disabilities who are experiencing homelessness or at risk of homelessness. This includes but is not limited to older adults (65+), youth transitioning from foster care, and veterans. ~Eligibility for Medi-Cal (138% Poverty Level; income of $16, 395 for an individual)
Service Locations • Co-located in 14 GR District Offices • Community based locations in each SPA • Jail facilitates only located in SPA’s 2, 4, and 6 • Benefits Advocate teams are mobile

Clinical Team 01 02 Link patients to primary medical health home Obtain medical records from all treating facilities 03 Assist teams to construct a clinically coherent medical summary 04 Obtain medical summary report from primary treating physician
Role of Clinics Provide medical evidence for disability claim • • Documentation for disability applications Work with CBEST advocates to complete medical summary report Train clinicians • DHS team will be developing curriculum for clinical staff to train providers • Blue book listings • Documentation for disability claims
How to Refer into CBEST Electronic Referrals • • Department of Public Social Services Coordinated Entry System Leads Housing for Health Contracted Providers Department of Health Services Hospitals and Clinics Fax Referrals • Community organizations can refer potential applicants via a faxed referral form. Please contact the agencies above for instructions. Appointments and Walk-ins • Potential applicants can schedule an appointment or walk-into the community based locations during walk-in hours. Please contact the agencies above for instructions.
Success Story 40 year old male, diagnosed with paranoid schizophrenia and experiencing homelessness for the past 15 yrs. CBEST advocates worked with DPSS to connect him to GR and Cal. Fresh within 1. 5 hours. Medi-Cal has been approved and we have already connected with DMH to begin treatment and documentation for an SSI application.
