Sustainability • The capacity to maintain program services at a level that will provide ongoing prevention and treatment for a health problem after termination of major financial, managerial and technical assistance from an external donor. – Entire service may continue under its original or an alternate structure; – Parts of the service may continue; or – May be a transfer of some or all services to other service providers • Institutionalization
Health Enterprise Zones • Born out of MD Health Quality and Cost Council’s Health Disparities Workgroup • Policy framework was enabled by the MD Health Improvement and Disparities Reduction Act of 2012 • Goals: – Reduce disparities and related costs – Improve access to care – Improve community-level interventions to broaden the scope of care and address social determinants of health – Improve health outcomes
