BREASTFEEDING IMMUNIZATION POLICY Provide a state-level support system to improve care for breastfeeding moms and babies. Promote immunizations towards an 80% goal as an important way to keep kids and families healthy. Ensure that laws, policies and programs work to support the health of children, families, and our communities.
CHILD DEVELOPMENT HEALTH CARE FOOD ACCESS Provide developmental screenings free of cost so that families can monitor their child’s physical and social-emotional development. Connect people access the state with the health care resources they need, to promote long-term health and wellness. Connect people across the state with food resources that help them access better nutrition.

Client Information As subcontractors with DSHS it is very important to maintain client records confidentially. • Keep paper records (names, DOB, SSN) filed away and locked when not in use. • Electronic files should be stored on a secured server where only authorized personnel have access.
Client Consent Forms When outreach staff complete applications they must get a signed consent form from the client. • On Washington Connection this can be done as an e-signature • For paper applications please use a copy of the consent form 14 -012

Non-Disclosure Forms ALL staff who have access to client information are required to sign Non-Disclosure Forms annually. • This includes staff who submit online applications as well as paper. • The forms must be kept on file at your office • Jefferson will review these forms at your onsite meeting
