Major Incident [All Paper] A large number of patients arriving on mass and need to be booked in and treated quickly • • • IT to supply preregistered MRN numbers for use in Major Incidents. ED to set up Major Incident Packs A sheet of patient labels Wristbands x 2 Downtime paperwork Patient identification sheet Observations chart X ray request x 2 Pathology request x 1 Transfusion x 1 Property bag marked with the label – Patients will be allocated and known by the preregistered MRN numbers provided throughout their care. – Wristbands would display the preregistered details – Pathology and Radiology would be requested against the preregistered MRN – Blood Transfusion? – Once Discharged there would be reconciliation with their actual record.

Downtime Forms [All Paper] Lorenzo and/or other hospital systems are not available v If Patient is known to trust v Patients “booked in” on ED paper downtime forms v Wristbands would show the patients actual details. v ED would use a manual whiteboard v Pathology and Radiology would be requested on ICE if the patient was known to the trust (and ICE was still available ) v If Patient not known in ICE v If the patient was not known to the trust, Pathology and Radiology would be requested on paper. v Blood transfusion? (clarification required) v Once Discharged there would be reconciliation with their actual record. v If Patient is unconscious and/or department are unable to retrieve patient details v If patient is unconscious and unable to give details, patient to be registered on a Downtime form using the not known naming convention: v Airddmmyyyyhrmin v Walkddmmyyyyhrmin v Roadddmmyyyyhrmin v v v If an unknown patient arrives by ambulance on 9 th March at 2 pm you would enter Road 090320151400 Gender if known or “not known” if the gender cannot be determined. The DOB 01/01/1900 Pathology and Radiology would be requested on paper. Blood transfusion? (clarification required) Once Discharged there would be reconciliation with their actual record
