Recognize when to use modifiers when coding orthopedic services. 3. Determine intraoperative services and imaging guidance work for orthopedic procedures and know when they are separately reportable. 4. Define common musculoskeletal system surgical techniques. 5. Understand the physical structure of the spine and its coding guidelines. 6. Describe the guidelines for coding fractures and explain the correct assignment of casting/strapping codes. 7. Describe the guidelines for the use of codes in the endoscopy/arthroscopy section of the musculoskeletal surgery codes in CPT. 8. Assign codes for common shoulder, elbow, and wrist procedures. 9. Discriminate between flexor and extensor tendons of the hand, describing common tendon repair procedures. 10. Assign codes for hip arthroplasty based on the components used. 11. Steps in Coding Fractures and Dislocations Follow these decision steps to assign the appropriate fracture care code: 1. What is the site of the fracture or dislocation? 2. Is the treatment open or closed? 3. Was a reduction or manipulation of the fracture performed? 4. Was traction used at the same fracture site? If so, it is not coded separately and is considered part of the fracture care. Traction of another fracture site may be separately reported. 5. Was an internal or external fixation device applied? Copyright © F. A. Davis 2018 6. Was percutaneous pinning or skeletal fixation applied? 7. Was infection present, treatment delayed, or did the surgery take longer than usual? 8. Was electrical stimulation (to promote bone healing) used? If so, assign codes 20974 or 20975 plus a HCPCS code for the actual stimulator. 9. Was computer-assisted imaging used to navigate during the procedure? 10. Is this a re-reduction or an initial treatment? If this is a re-reduction in the global period, assign the appropriate modifiers. 26
