Leadership Team Co. C Coordinator & Lead Agency Lead entity composed of representatives from the following municipalities: • Independent Cities: Harrisonburg and Winchester • Counties: Clarke, Frederick, Page, Rockingham, Shenandoah, Warren Leads the Co. C Coordinated Application process, promoting regional cooperation, partnerships, and knowledge sharing, and serves as a intermediary between the Leadership Team and Western Virginia Co. C. Team facilitates and coordinates community’s efforts to prevent, treat, and end homelessness across the region. Western Virginia Continuum of Care (Co. C) Coordinated by the Co. C Coordinator & Lead Agency and composed of organizations that provide services to the homeless population, including prevention efforts The Co. C facilities the planning process that are the primary tool for addressing homelessness, such as Built for Zero Executive Committee Comprised of the chairs of each standing committee, the chairs of each local planning group, and a representative from each lead agency. This committee is responsible for administrative oversight of the Co. C and is empowered to act on behalf of the Co. C in between meetings. Best Practices Responsible for researching and implementing best practices for meeting the needs of people experiencing homelessness in the region, establishing a Co. C coordinated assessment system, creating and implementing written standards for administering assistance, assessing regional training needs, and facilitating training opportunities. Built for Zero/ Case Conferencing Meets on a consistent basis to review the by-name list, work collaboratively to address needs of homeless individuals in our community, and assess progress in reaching functional zero homelessness. Also, annually review and update case conferencing assessment tool. Compliance & Evaluation Grant Evaluation Media & Advocacy Comprised of non-grantee representatives, this committee conducts quarterly evaluations on Co. C and HSNH grantees, based on standards established in the grants, provide feedback and necessary action steps to meet compliance, and reviews and standards on an annual basis. Comprised of non-grantee representatives, this committee establishes grant application assessment, conducts application assessments for the Co. C and HSNH sub-grant proposals, and submits the rank and recommendation of the Works to increase the visibility of the Co. C by developing messages about homelessness to be used in the media, drafting press releases, posting on social media, and pitching stories to the media. A media plan will be created and reviewed each year.
