Description of Event • Near miss occurred while installing a compressed air manifold, near the Ross Shaft at the 5000 L. • The subcontractor was installing a compressed air manifold system. • After installing and leak checking, the first leg of the system, a subcontractor employee bumped the closed air valve handle, causing compressed air to be released from the end of the hose blowing gravel into the air. • A subcontractor employee was struck in the face with gravel causing a light abrasion on the face. • The subcontractor on-site paramedic evaluated the individual and no medical treatment was required. 2 2/11/2022 Presenter | Presentation Title or Meeting Title
Resolution • Air line was bled-down and locked-out of service until the manifold installation was completed. • The valve was also replaced with mechanical locking valve. 3 2/11/2022 Presenter | Presentation Title or Meeting Title
Incident and Issues • The subcontractor was installing a compressed air manifold system. The subcontractor on-site paramedic evaluated the individual and no medical treatment was required. • After installing and leak testing, it was noticed that the existing facility air system was leaking which caused the subcontractor crew to test the manifold system in segments to identify the location of the air leak.
