APPLICATION FORM • WDNR Form 3400 -053 • Obtain from: – Local DNR Service Center – DNR Website • www. dnr. wi. gov • Insert “ 3400 -053” into Search Window • General – 4 page form. – Pages 1 & 2 need to be filled out – Pages 3 & 4 provide directions

SITE INFORMATION • Provide the OUTLINE of the SITE on the soils map/aerial photo • Name the SITE – Name the individual fields on the site • Provide Legal Description and Acreage • Owner / Farmer if different • Entity applying for site

SITE INFORMATION • Do you comply? ? – Separation Distances, – Check the box • Comments • Print name & title, signature and date

PAGE 2 • Existing Cover Crop • Is the site being used for non-agricultural waste? –check the box • Adjacent land uses—check all that apply • Vertical separation: bedrock/groundwater – Check the box • REVIEW the setbacks—Septage on the bottom portion of page

SITE APPROVALS • DNR Staff is backlogged this time of year. • May take more than 7 days formal approval. • If you need one site more than another one, communicate that to the Regional Septage Coordinator

SITE APPROVALS • You can use a site 7 days after submitted IF IT IS AN APPROVABLE SITE. If you mess up on the application, the site fails to meet minimum standards, or simply state false information AND you use the site – Expect a CITATION!
