Kristin Drouin, LCSW, Fellow Lexy Morvant, MD, Fellow Jeanne Macdonald, MD, Fellow Boston Children’s Hospital & Dana-Farber Cancer Institute Management of Fellow Primary Patient Panel Follow-up Tasks in Pediatric Palliative Care AIM: By January 1, 2018, PACT Fellows aim to increase completion of weekly assigned tasks from roughly 60% to 80%. INTERVENTION: We created a process for reviewing and managing patients in our primary patient fellow panel. However, we realized that task creation/completion did not translate into quality palliative care for our patients. NEXT STEPS: We now hope to measure the quality of the care that we are providing through family surveys § We hope to learn more about how easily this process is transitioned to the next fellow class § We hope to add a triage system to our patients that is agreed upon by the Core team to help with standardizing quality palliative care by all PACT patients § Continue to receive input from Core members and faculty to help with panel quality evaluation
