INTERVENTION: We created a process for reviewing and managing patients in our primary patient fellow panel. We measured task completion % as we added more structure to our panel process through multiple PDSA cycles (created excel sheet, sent email summary for reminder of tasks, reviewed EMR, protected time. ) § § § RESULTS: Graph title Insert graph TEAM: Pediatric Advanced Care Team § Marc-Antoine Marquis, MD § Nicole Fernandez, NP § Jenna Freitas, NP § Rachael Burgess, NP § Marsha Joselow, LICSW § Nick Purol, LICSW § Shih-Nang Liaw, MD PROJECT SPONSORS: § Emma Jones, MD CONCLUSIONS: Our aim was met by increasing our task completion percentage to 80 -100%. However, we realized that task creation/completion did not translate into quality palliative care for our patients. NEXT STEPS: We now hope to measure the quality of the care that we are providing through family surveys § We hope to learn more about how easily this process is transitioned to the next fellow class § We hope to add a triage system to our patients that is agreed upon by the Core team to help with standardizing quality palliative care by all PACT patients § Continue to receive input from Core members and faculty to help with panel quality evaluation
