Who are our Grizzly Cubs? �Students attend Gilchrist based on their home address. Use the School Zone Lookup Tool on the LCS website to find your home zone school. �For School Choice and Reassignment options, please visit the School Choice Office page on the LCS website.

How do I register my child? �For the 2020 -2021 school year, new students to Leon County Schools will be registered online at: https: //www. leonschools. net/parentportal � If your child attended a LCS Pre-K program, you do not need to re-register your child for Kindergarten! � If you have been notified that your child is approved for School Choice, you do not need to re-register your child for Kindergarten!

What do I need to submit to Gilchrist? �The documents on the following slide should be submitted electronically to our Registrar, Ms. Chetta Haverty, via email: havertyc 2@leonschools. net or can be submitted in person once school re-opens per CDC/LCS guidance. � School Choice and former LCS Pre-K students must also submit this documentation! � After these documents are scanned into the Gilchrist database, they will be deleted/shredded.

Documentation to be submitted to our Registrar � Driver’s License of parent or legal guardian � Student’s Birth Certificate � Current Florida Health Physical Form Contact the FLDOH or your child’s physician for this form. It must be signed by a Florida healthcare provider and dated within 1 year of the first day of school. � Current Florida Immunization Form DH 680 Contact the FLDOH or your child’s physician for this form. � Two current proofs of your address One from each category: � Mortgage statement or rental/lease contract, Homestead Exemption, Property Tax Record � Current electric bill (preferred), home insurance bill, vehicle registration � Court documents pertaining to custody, if applicable
