Student and Special Education Snapshot Templates Student Template record must exist in the current school year for the LEA reporting the student in the Special Education Snapshot. ***ALL special education students ages 14 and older as of July 1, 2016 AND any other special education students with a transition plan as part of their IEP*** If any of these students also exited special education, report their exit date and exit reason. No other special education students should be reported in this collection. Examples 12 year old student with no transition plan that returned to regular education would not be reported. 14 year old student that did not exit special education during the school year would be reported regardless of whether or not the student has a transition plan. 13 year old student with a transition plan that exited special education would be reported including exit information.

Field 38= Y for all other records regardless of exit reason.

July 2017 Special Education Collection Other Considerations Can a student 14 or older as of 7/1/16 have an N in IEP Transition Status Code (45)? – Yes Can the LEA answer Y to field 45 and N to 73 -76? - Yes Moved within PA, known to be continuing – Optional Code 99 will be added for the 1718 SY 1617 SY - students that moved within PA that are 14 or older or have a transition plan, you have two options. 1. Keep them in the submission, and remove the exit date and exit reason 2. Remove them completely from the submission. In future years they can remain in the collection with code 99 and PDE will remove them from the LEA submission.

July 2017 Special Education Collection TABLE 9 A Continued… Column 5 is a report of disciplinary removals. In column 5 A, report the number of times any child with a disability was subject to any kind of disciplinary removal during the school year. When counting disciplinary removals, include in-school suspensions, out of-school suspensions, expulsions, removals by school personnel to an interim alternative educational setting for drug or weapon offenses or serious bodily injury, and removals by hearing officer for likely injury to self or others. Note that column 5 A is a report of disciplinary events, NOT, children. If a child has more than one disciplinary removal in the school year, then each removal should be reported in column 5 A.
