How can you access us? Speech and Language Therapy • Majority of referrals to our service come from the school – School complete a ‘Quick Communication Checklist’ which helps us identify specifically the concerns – School discuss the potential referral with their link SLT – The link SLT then advises whether a referral is indicated. There may be times where the SLT suggests alternative action (i. e. referral to another agency, suggestions for school-based support) • We do sometimes receive referrals from other professionals, such as GP, Paediatrician, health visitor, OT etc – In these instances, we make contact with the school to gather information regarding the child’s communication skills in the school context.
When do you leave us? • Children will be discharged from the service for the following reasons: – Therapy goals are achieved – Ongoing support can be carried out at home/school independently – Transition to adult services – Transition to another service – Non-attendance of appointments – Non-response to request for further information • Families and GP will receive a discharge letter with summary of intervention, advice and re-referral details if necessary
