End-of-Placement Evaluation • fill in Appendix 5. 15 (endof-placement) through the web. • complete Appendix 5. 9 by instructor through the web. • complete Appendix 5. 10 by student through the web. • complete Appendix 5. 13 by agency (File) • compile student digital portfolio (Full)
Some reminders. . . : Student Digital Portfolio should be uploaded onto the Web within 1 week after the placement ends : Appendix 5. 15 is confidential. No need to show it to your instructor. They cannot read it. : They will only know the result after the announcement of results. : instructor ’s mark/grade is a recommendation only. Subject to review. : Grade-related special requirements & ‘D’ cases : Your signature in the form does not necessarily mean that you agree with the mark recommended!
