Introduction Male infertility는 몇몇의 유전적인 요소와 비유전적인 상태와 관련이 있다. Genetic abnormality 1. 수적 구조적 염색체 비정상 2. Y 염색체의 AZF region의 deletion 3. CFTR의 mutation CFTR(cystic fibrosis transmembrane conductance regulator): -다른 transport pathway 조절 -chloride channel로의 역할 -이 유전자가 mutation 되었을 경우 *낭포성의 섬유형성이나 선천적으로 수정관의 양측면 발육부전증 (CBAVD-congenital bilateral absence of the vas deferens)이 될 수 있음

Materials & Methods 최소 일 년 동안 불임을 지닌 ICSI(미세조작을 통한 시험관 아기시술)이 요 구되는 couple로 부터 150명의 남자 선정 Semen analysis : < 1 X 106/ml(WHO: world health organization) guideline에 일치하게 실시 Cytogenetic analysis: FISH(fluorescence in-situ hybirization) *probe: cp 9 -23. 1(X/Y chromosome short arm telomere region) p. DP 105(Y chromosome testis determining) Y chromosome long arm AZF region sub-microdeletion - multiplex PCR으로 분석 *Y chromosome specific marker AZFa: DYS 148, DYS 273, AZFb: DYS 218, DYS 224, AZFc: SY 245, SY 255(DAZ gene) CFTR gene 12개의 common mutation: F 508, A 445 E, G 542 X, 17171 G A, R 117 H, R 1162 X, N 1303 K, W 1282 X, 3659 del. C, E 60 X, S 125 IN

Results Azoospermia: 37명(24. 7%) Severe Oligozoospermia: 113명(75. 3%) 이 중 36명(24%)의 genetic abnormality 환자 동정

Genetic abnormality Abnormal karyotypes(10. 6%): 16/150 AZFc deletion(5. 3%): 8/150 CFTR gene mutation(9. 3%): 14/150 Abnormal karyotypes 9명- sex chromosomal aneuploidy 6명- translocation 1명- marker chromosome 발견 Y chromosomal deletion Testicular insufficiency로 인해 male infertility와 주로 연관되어 있다. 모든 deletion은 DAZ gene을 포함하는 AZFc region에서 동정되었다. CFTR gene mutation 보통 mutation되었을 경우 수정관의 선천적인 부재가 발생하지만 16% 정도는 수정관의 이상이 없는 Azoospermia 였다.

Oligozoospermia(sperm cell의 수 감소), asthenozoospermia(sperm 운동성의 감소), tetratozoospermia (sperm 형성 시 비정상적인 형태 생성), oligo-astheno-teratozoospermia: OAT syndrome(asthenozoospermia와 tetratozoospermia의 결합된 상태)

Discussion Oligozoospermia, asthenozoospermia, tetratozoospermia, oligo-asthenoteratozoospermia syndrome과 같은 male subfertility의 병리 특발성 질환성 형 태는 환경오염과 활성산소 그리고 유전적인 비정상으로 인해 나타난 내분비 장애와 같은 요인에 의해 설명되어져왔다. 본 연구에서는 male infertility를 표면적으로 탐지할 수 없었던 몇몇의 genetic risk factor(선천적인 염색체 비정상, Y 염색체의 결실, CFTR gene의 mutation)과 관련 지어 보았다. Extreme oligozoospermia와 azoospermia의 36/150(24%)가 genetic abnormality였다. Abnormal karyotypes(10. 6%): 16/150 Numerical sex chromosomal abnormality는 infertile male에서 cytogenetic abnormality의 63%였으며 보편적으로 azoospermia에게서 발견되었으며 Robertsonian과 reciprocal translocation과 inversion은 oligozoospermia의 남 성에게 나타났다. 그리고 X chromosome이 하나 더 있는 Klinefelter sydrome은 germ cell atresia(폐쇄증)과 관련되어 있었다.

AZFc deletion(5. 3%): 8/150 Y chromosome의 AZF-a, -b 그리고 –c 영역에서의 DNA 분석의 결과는 azoospermia(DAZ) gene을 포함하는 AZFc 영역에서 모두 deletion되었다. Y chromosome deletion은 최근 spermatogenetic failure의 원인이 되는 가장 보편적인 남성불임의 유전적인 요인이다. 이러한 deletion은 불임남성의 3 -21%정도를 차지한다고 보고된 바 있다. 그리고 severe oligozoospermia와 non-obstructive azoospermia, normal physical finding, gonadotrophin의 정상 농도와 관련되어 있었다. CFTR gene mutation(9. 3%): 14/150 CFTR gene의 mutation은 cellular chloride excretion과 pancreatic function의 장애로 나타 나며 낭포성의 섬유형성이나 선천적으로 수정관의 양측면 발육부전증(CBAVD-congenital bilateral absence of the vas deferens)을 나타내기도 한다. CFTR gene의 mutation은 gene의 non-coding region에 8번 intron 안의 polypyrimidine과 같은 alteration을 일으켜 9번 exon의 skipping translation으로 인해 일어나며 그 결과 CFTR protein의 비정상적인 level이 나타나고 CFTR protein의 농도는 감소된다. 결론적으로 CFTR gene의 mutation은 보편적으로 primary testicular failure 보다는 male genital tract의 방해와 관련되는 CBAVD와 관련이 있으나 10/144명의 남성은 관련 없었다. 그 리고 최근에는 CBAVD를 지닌 일부의 남성이 defective spermatogenesis를 보여주기도 했다.
