Introduction ‘We help our customers protect patients from harm by creating opportunities to learn from things that go wrong. ’ As well as providing patient safety software and tools. Datix is also well known for supporting, engaging and been a leading voice in the global patient safety community. This provides Datix with a strong platform from which to move into a more prominent role in supporting culture change and patient safety improvement globally.
Why is a new body needed? § Healthcare organisations around the world have an inconsistent approach to addressing culture and delivering sustained patient safety improvement § Datix’s international standing will enable us to fulfil a unique role that no other single body is able to bring together
What can Datix offer? § Able to bring together a global perspective from some of the largest and most prestigious healthcare organisations internationally § More than 30 years’ experience working in the patient safety field, with connections to an international network of patient safety leaders and experts. § Our customers! Changing perceptions… “Incident reporting is the single biggest waste of time in the last 15 years and it is the most mistranslated intervention from aviation. ” - Kaveh Shojania - Director for the Centre for Quality Improvement and Patient Safety at the University of Toronto § Negative perceptions surrounding incident reporting exist – largely due to poor systems, implementation and culture. § The new Patient Safety Institute will play an important role in sharing patient safety improvement derived from reporting systems, reinforcing the link between incident reporting and protecting patients from harm and changing negative perceptions.
Activities § Providing online tools for sharing learning from things that go wrong (linking to Datix IQ) § Commissioning, sponsoring and sharing research on culture and behavioural change and patient safety improvement § Working with healthcare organisations to share experience and best practice in patient safety improvement and culture change
Activities § Conferences and seminars to share knowledge, tools, and experience § Working with organisations to improve incident reporting and learning § Developing and promoting high quality incident investigation and embedding learning into policies, procedures and practice § Newsletters, blogs, articles in journals § Working collaboratively and in partnership with other patient safety focused organisations
Consultancy § Providing expert patient safety improvement advice and support to healthcare organisations § Leadership and culture change § Evidence based quality improvement science § Emphasis on measurement and evaluation § Sharing learning and experience
Incident Investigation Training § Providing high quality incident investigation training to organisations § Accreditation (perhaps through HSIB) § To include: - Investigation methodology - Developing strategies and interventions - Evaluation and monitoring - Promoting a ‘just’ culture - Supporting staff and families
A focus on improving safety learning culture across all healthcare organisations. § No matter how good the software tools Datix provides to its customers, success in protecting patients from harm through reporting and learning systems will always be limited by poor culture and a lack of associated resources and expertise
Principles § Politically neutral. § Arms length from Datix § Evidence based § Not for profit. Any income generated will be reinvested in to support further work of the institute § Feedback and evaluation on all aspects of the institutes work and activities will be sought and used to drive continuous improvement
