Project Introduction • Overview of service provided This project will provide an environment where we can understand how individuals prefer to communicate and provide them with individualised packages so that they are able to make sense of the world they live in and play an active role in their lives and their communities. • Staff structure • 1 x Band 7 Occupational Therapist

Project key achievements Key outcomes and Performance Indicators • Allow individuals to make choices and express themselves • Promote individuals understanding of the world and allow them to communicate in a way which we do not find challenging • Promote fair and equal access to services and the community by reducing barriers, • Provide the right environment within the person’s community of origin decreasing the need for out of county specialist placements. • Promote the longevity of family living decreasing the need for residential placements and subsequent hospital admissions. • Ensure local community provision is robust enough to support a community based step down package by providing skilled and confident staff with the right resources in the right place. • Ensure the least restrictive option of support is available, which is focused on individual’s strengths.

Sustainability plan • How will the project be sustained in the future? Evaluation will take place in quarters 2 and 3 of final year and if evidence confirms the effectiveness of the approach this will then be figured in to service redesign to allow core provision to continue delivery.
