This is done to avoid medication errors such as omissions, duplications, dosing errors, or drug-drug interaction. • 1 --Develop a list of current medications. • 2 --Develop a list of medications to be ordered/prescribed by the admitting physician. • 3 --Compare the medications on the 2 lists. • 4 --Make clinical decisions based on the comparison. • 5 --Communicate the new list to appropriate caregivers and the patient.

This is USP 797. • Horizontal laminar flow hood • Biological safety cabinet(vertical flow hood for chemo agents) • Glove box • These need HEPA filter • ISO class I is cleanest room • Tech must initial on label, pt name, pt record #, room #, drug name, strength, name of mixed solution, infusion rate, does time, date made, exp date • Expired IV are logged out, disposed.
