Initiële opvang bij cervicale traumata Instructional Course Lecture Belgian Spine Society H. Robijn, GSO Orthopedie, KUL
opvang cwz trauma ≠ immobilisatie en radiologie Initiële opvang cervicale traumata - BVOT 2008
Extramuraal • Primaire evaluatie en resuscitatie: ‘ABCD’ – Hypoxie – Shock • Hypovolemisch • Distributief: verlies sympatische tonus • Secundaire evaluatie – Neurologie • Stabilisatie/immobilisatie • Extirpatie en transport Initiële opvang cervicale traumata - BVOT 2008
Secundaire evaluatie CWZ • Klinisch nazicht – Trauma pathogenese Initiële opvang cervicale traumata - BVOT 2008
– Symptomatologie • Bewustzijn • Pijn • Neurologie: spinaal letsel? – Compleet vs Incompleet – Motorisch Flabouris 2001: tot 50% zonder lesie maar abnl neuro sensibele sympt alleen: 27% -lesie vs 8% +lesie • Hoofdtrauma? Yanar 2007: geen hoofdtrauma, geen spinaal letsel I (CSI)=2. 1% (>8400 voetgangers) – Klinisch protocol cwz Initiële opvang cervicale traumata - BVOT 2008
Risicofactoren cwz • Hoog energetisch • Kans op instabiliteit • Andere letsels: – Yanar 2007: associatie met • • Hoofd Thorax Pelvis Femur • Leeftijd • Traumatype Initiële opvang cervicale traumata - BVOT 2008
Traumatype • Penetrerend – – Minder frekwent Europa Vuurwapens, scherp voorwerp (40%-44%) Vasculair trauma (21%), ht/pt (18%) aerodigestief (6 -9%), RM zz • Stomp – Veel frequenter – vko (motor!) – Musculair/ligamentair/osseus trauma: instabiliteit Initiële opvang cervicale traumata - BVOT 2008
Stabilisatie CWZ: noodzakelijk? • Pro: – Preventie bijkomende letsels (instabiliteit) – Verzekeringstechnisch? Bewijs? • Contra: – Overbehandeling – Oncomfortabel: agiteert – Creatie bijkomende letsels bij aanleggen – Maskeert de nek Bewijs! Initiële opvang cervicale traumata - BVOT 2008
• Immobilisatie niet nodig als: – Stabiel – Complete dwarslesie (tetraplegie) • Penetrerend vs stomp: – zz instabiliteit (compleet SCI) – levensbedreigende letsels (vasculair/luchtwegen) » » Expanderend hematoom Tracheadeviatie SC emfyseem ↓ Carotispulsaties Initiële opvang cervicale traumata - BVOT 2008
• Barkana (2000): 54 penetrerende CWZ lesies (Israel) – geen instabiliteit – 22% (!) symptoom van levensbedreiging • Carducci (1986): meta-analyse 1830 cervicaal penetrerend – – 40% majeur vasculair 18% tracheo-oesophagaal 2. 7% cwz 1. 9% ruggemerg • Arishita (1989): 472 penetrerende cervicale lesies (Vietnam) – 3. 7% (11) cwz – 1. 4% (4) instabiel Initiële opvang cervicale traumata - BVOT 2008
• Waarom géén immobilisatie? – Hoofdpijn/rugpijn → radiologie – Druknecrose – Belemmert respiratie → hypoxie/agitatie – Manipulatie: zo aggressief → SCI – Nota kind: cave hoofdextensie op spineboard Initiële opvang cervicale traumata - BVOT 2008
Klinisch protocol/decisie-instrument • Hoffman e. a. (NEJM 2000): geen immobilisatie als • • • no midline posterior tenderness no focal neurologic deficit normal alertness no intoxication no painful, distracting (other) injury – Prospectief: 34. 069 ptt – n=818 cervicaal spinaal trauma: n=578 klinisch significant – Sensit 99% (n=810), klinisch significant: sensit 99. 6% (n=2) – Negatief predict waarde 99. 8% / 99. 9% Initiële opvang cervicale traumata - BVOT 2008
• Domeier e. a. (Ann. Em. Med 2005): – Idem: • • • – – no midline posterior tenderness/pain no focal neurologic deficit normal alertness no intoxication no painful, distracting (other) fracture Prospectief: 13. 357 ptt 3% (n=415) spinaal trauma: 128 CWZ – 50 SCI (12%) T 12, L 1, L 2 > C 1, C 2 – C 5, C 6, C 7 Sensit protocol 92% (‘missed’: L) Initiële opvang cervicale traumata - BVOT 2008
• Burton e. a. (J Trauma Inj Inf Crit. C 2006): – – Prospectief, 31. 885 traumatransporten 12. 988 (41%) met EMS protocol immobilisatie 154 fracturen 20 (13%) zonder immobilisatie: 1 instabiele thoracale fractuur • • Mentale status Pijn van ander letsel Perifeer neurologisch onderzoek Pijn/gevoeligheid ruggewervels – Sensitiviteit 87% (unstable 94%) – Negatief predictieve waarde 99. 9%! Initiële opvang cervicale traumata - BVOT 2008
• Belang van klinisch protocol: – Hoffmann: • 34. 000 patienten • 800 CSI • 29000 klinisch + , radiologisch – • 4300 klinisch - , radiologisch → +/- 13% minder radiologische onderzoekingen: economische consequenties stralingsgevaar Initiële opvang cervicale traumata - BVOT 2008
Pharmacologie • Macroscopisch trauma • Microscopisch trauma: cellulair niveau – NASCIS schema • 2 e (1990) en 3 e (1997) studie MPSS • Prospectief, gerandomiseerd multicenter, double-blinded, placebo-controlled • Initiële bolus 3 -8 u posttrauma (>8 u: slechtere uitkomst!), 48 u iv • Effect tot 6 -12 maanden later Initiële opvang cervicale traumata - BVOT 2008
Conclusies Ø Extramurale trauma-opvang: initiële resuscitatie, secundaire evaluatie en evt stabilisatie Ø Stabilisatie CWZ niet altijd gewenst! Ø Ø Ø Klinisch protocol CWZ: zo goed uitgevoerd betrouwbaar instrument Stomp versus penetrerend trauma Independente risicofactoren voor CWZ letsels: Ø Ø Ø Hoofd – thorax – pelvis – femur Leeftijd Gebruik bij aangetoond letsel NASCIS schema (1 e dosis 3 -8 u posttrauma) Initiële opvang cervicale traumata - BVOT 2008
