Rationale for COVID‐ 19 PPE: N 95 respirators do not provide added benefit over medical masks for HCW during non‐aerosolizing procedures STUDY TITLE Long Y, Hu T, Liu L, et al (2020). STUDY DESIGN, SETTING AND PARTICIPANTS A systematic review and meta‐analysis to assess the effectiveness of N 95 respirators versus surgical masks for prevention of influenza. A total of six RCTs involving 9, 171 participants were included. Bartoszko, J. J. , Farooqi, M. A. M. , Alhazzani, W. , & Loeb, M. (2020). Systematic review to compare medical masks to N 95 respirators in preventing laboratory‐confirmed viral infection and respiratory illness including coronavirus specifically in healthcare workers. Four RCTs were meta‐analyzed adjusting for clustering. CONCLUSION The use of N 95 respirators compared with surgical masks is not associated with a lower risk of laboratory‐confirmed influenza. It suggests that N 95 respirators should not be recommended for general public and non‐high‐risk medical staff who are not in close contact with influenza patients or suspected patients. 1‐ Low certainty evidence suggests that medical masks and N 95 respirators offer similar protection against viral respiratory infection including coronavirus in healthcare workers during non– aerosol‐generating care. 2‐ Preservation of N 95 respirators for high‐risk, aerosol‐generating procedures in this pandemic should be considered when in short supply.

Steps for Donning COVID‐ 19 PPE 1. Before starting the donning process, make sure you have hydrated, used the bathroom and change from your personal shoes into gumboots. For staff with long hair/large hairdos and not requiring a head cover (see step 9), put on a hair cover. 2. Identify and gather the correct size/fit of PPE to don. 3. Perform hand hygiene. 4. Put on first pair of gloves 5. Put on isolation gown. Tie all of the ties on the gown. Assistance may be needed by another HCW. If there is a thumb hook sewn into the wrists of the gown, secure the gown with your thumb to avoid slip ups that could expose arms. 6. Put on medical mask (or NIOSH‐approved N 95 respirator if managing a patient with aerosol‐generating procedures). Both your mouth and nose should be protected. [*Do not wear respirator/facemask under your chin or store in scrubs pocket between patients. ] v Medical mask: Mask ties should be secured on crown of head (top tie) and base of neck (bottom tie). If mask has loops, hook them appropriately around your ears. Facemask should be extended under chin. v Respirator: Respirator straps should be placed on crown of head (top strap) and base of neck (bottom strap). Respirator should be extended under chin. Perform a user seal check each time you put on the respirator. If the respirator has a nosepiece, it should be fitted to the nose with both hands, not bent or tented. Do not pinch the nosepiece with one hand. 7. Put on reusable face shield or goggles. 8. If you are using a gown that is not fluid‐resistant, don a disposable apron before proceeding to the next step. 9. If managing a patient with aerosol‐generating procedures, put on head cover (e. g. , surgical bonnet) over face shield/ goggles (see instructional slide) 10. Put on second pair of gloves. Gloves should ideally have a cuff long enough to cover the cuff (wrist) of gown. 11. Have a colleague check you to ensure that have donned correctly before entering the patient care area

Do so in gentle manner, avoiding a forceful movement. v Reach up to the shoulders and carefully pull gown down and away from the body. v Remove gown allowing the outer pair of gloves to be removed at the same time. Dispose in the waste bin 3. Perform hand hygiene. 4. Remove head cover if wearing Level 4 PPE (see instructional slide) 5. Remove face shield or goggles. Carefully remove face shield or goggles by grabbing the strap and pulling upwards and away from head. Do not touch the front of face shield or goggles. Place the goggles/face shield in a designated reusable PPE receptacle. 6. Perform hand hygiene 7. Remove and discard medical mask (or N 95 respirator). Do not touch the front of the mask. v v Medical mask: Carefully untie from back of head (or unhook from the ears) and pull away from face without touching the front. Respirator: Remove the bottom strap by touching only the strap and bring it carefully over the head. Grasp the top strap and bring it carefully over the head. Pull the respirator away from the face using the top strap (without touching the front of the respirator) and discard in the appropriate waste bin. 8. Perform hand hygiene on inner gloves. 9. Remove hair cover and shoe covers (where applicable). Perform hand hygiene after removing each element 10. Remove inner pair of gloves avoiding touching bare skin with potentially contaminated areas of the gloves (use the clean‐to‐clean and dirty‐to‐dirty technique). 11.
