Enter the name of the person completing the form Enter the email address of person completing the form Enter the telephone number of person completing the form Enter the total grant you have been issued for Infection Control Round 2 Select the month for which you are reporting e. g. for reporting period 1 st October - 30 th November please select November 2020

Select from the drop down whether you are a residential provider or community care provider as this will take you to the correct set of questions. Enter the name of your home Enter your Location ID Answer the first question, if you select yes it will take you to two further questions

Input how much you have spent in the reporting period you are reporting for Input how much you anticipate your total spend to be over the 6 month period. This needs to include your figure in the previous question which will provide a cumulative total If you select ‘No’ to this question, you just need to input how much you anticipate your total spend to be in this area

You will need to go through every question with either a yes or no answer Once all questions have been answered click ‘Submit’

Once you have clicked ‘Submit’ you will receive this notification which completes your monthly return. If you are completing multiple forms, please click on the ‘submit another response’ option.
