Origin of the AIDS pandemic in Congo 1. All the earliest AIDS cases from 1959 to the epidemic in 1979 are found in that area. Nearly all have been confirmed by serum testing. 2. The earliest pre-epidemic and epidemic of HIV-1 group M took place around that region. In 1978 the HIV-1 rate per 100, 000 was 15 in Kinshasa; 20 in Burundi; and 16 in Rwanda. 3. Resistencias y su interpretación • Los mutaciones y los patrones de resistencia en los diferentes subtipos del VIH siguen mayoritariamente los principios del subtipo B. • Sin embargo, la variabilidad genética determina que algunas mutaciones sean más frecuentes en función del subtipo y/o que la barrera genética difiera de un subtipo a otro.

Vacunas y Subtipos • El control real de la pandemia del SIDA es dependiente del desarrollo de una vacuna preventiva efectiva. • La diversidad genética del VIH-1 representa una de las mayores dificultades para alcanzar este objetivo. • Aunque existe reactividad cruzada en la respuesta inmune frente a diferentes subtipos, la amplitud de esa respuesta es limitada. • Uno de los desafíos mayores en el desarrollo de vacunas frente al VIH depende de la capacidad para incorporar la diversidad genética del virus, elicitar una amplia gama de anticuerpos neutralizantes y generar una respuesta efectiva CD 4/CD 8.

HIV-2 summary • HIV-2 infection in Spain remains mainly confined to immigrants from endemic regions. • Overall, lower virological efficacy and poorer CD 4 restoration in HIV-2 vs HIV-1. • Lack of activity of NNRTI and entry inhibitors. • PI susceptibility: SQV > ATV > DRV > LPV Only residual for f. APV and TPV. • Good activity of raltegravir. • Viral load reliable only for HIV-2 clade A. • Drug resistance mutations in HIV-2 are selected at the same positions than in HIV-1 but at different frequencies. • Caution in dually infected HIV-1 & HIV-2 patients.
