What is an IPOC? Individualized Plan of Care l For patients whose frequent visits need multidisciplinary care planning l For l To families needing guidelines be used for future ED visits or admissions
Purpose of an IPOC l l l To create a consistent formatted guideline for staff in addressing patient’s complicated needs To provide an avenue for clear communication of plan to ensure proper care of patient To provide consistency in handling patient needs To provide multidisciplinary expertise To alleviate staff dissatisfaction and improve patient satisfaction

Where do staff fit in? l Help identify patients needing plan (referral) l Utilize plan intervention in your daily work l Assist with passing of important information to next level of care at discharge or transfer l Provide feedback and suggestions to improve care

Process l Patient’s admission will trigger a page to the Administrative Supervisor l The Administrative Supervisor will retrieve plan and deliver copy to unit charge nurse or designee l Charge nurse will review plan with unit nurses and incorporate interventions in patient’s plan of care
Review of IPOC after Discharge l Design Team will meet with Unit staff, AS and other disciplines involved l Questionnaire will be filled by staff prior to meeting with team l Team will revisit plan, make changes with staff input. l Team will save plan for next visit
Goal l Improve staff and patient satisfaction l Provide consistency in providing quality patient care l Promote a positive patient and family experience
