Incident Flowchart (Accident, Near Miss, Violent Incident, Dangerous Occurrence & Disease) Are you reporting an incident and/or N violent incident? N Report as a near miss or Violent incident. This includes physical & threating violence A dangerous N occurrence** Y Y Was an injury sustained? Did it occur as per HSE definition? Y Was it a fatality? N Y Call Health and Safety Y Is disease reportable as per N HSE definition? N Y Was injury a reportable specified N injury? ** Y Was the injured Y person an employee? N Was the injured person an Y employee? Were they absent from work or N unable to perform their normal work duties for more than 7 days? Call Health and Safety Y Report as near miss Y N Inform Manager & Head of Service Were they taken to hospital Y DIRECTLY from the scene? N Call Health and Safety Adviser If the incident occurred Out of or in connection with work - the Adviser will notify HSE A near miss or fire incident? A disease **? N Call Health and Safety on 8 th day Report on form OHS 1 within 5 days of the incident occurring All reports to Health and Safety Remember! If in doubt contact Health and Safety *H&S will phone HSE (HSE RIDDOR reportable examples are detailed on the rear cover of the incident report pad) Over seven day absences are RIDDOR reportable and incident report forms must be submitted to Health and Safety on the eight day and no later than 15 days after the incident **See incident reporting guidance document on My Toolkit for further information on specified injuries, dangerous occurrences and diseases No action required seek further advice from Health and Safety
