Immunization Certificates • You should only request Immunization Certificates through the Portal • DO NOT request Certificates out of Bridge or GUI.
Patient brings in shot record • Before you enter the shots into CDP, check the registry first, they may have already been entered. • All babies born in the state of Kentucky since 2006 should have information in the registry.
Problems • Patient does not show in registry, and you know that they should have shots in the system • Contact Customer Support • 1 -866 -237 -4814
Problems • Patient has shots in the registry that do not belong to that patient. • If they were entered through Bridge or GUI you can remove those shots.
Problems • Patient shows shots in bridge/GUI but they do not show on the registry. • Bring up the Immunization History screen and re-transmit. That should send them over to the registry.
