Patient Specific Identifiers Using person specific identifiers to confirm that clients receive the service or procedure intended for them can avoid harmful incidents such as: • Privacy breaches • Allergic reactions • Discharge of clients to the wrong family • Medication errors • Wrong person procedures
Medication errors in hospitals is the most frequently identified area in the literature of patient safety Medication errors occur with: • misread orders • medication given to the wrong patient • doses being “doubled up” • doses being missed
Major Test for Compliance At least two person specific identifiers are used to conform that clients receive the service or procedure intended for them, in partnership with clients and families.

Use at least two of the following acceptable identifiers And check against or verify by Patient’s Full Name (first and last) • Requesting patient or responsible party to state and spell patient's full name • Patient ID wristband • Alberta Personal Health Card Patient’s Date of Birth • Requesting patient or responsible party to state patient's birth date. • Patient ID wristband Unique Lifetime Identifier • Patient ID wristband • Health record • Barcode • Alberta Personal Health Card Facial Recognition • Photo (eg. drivers license, health record photograph) • Conducting an independent double check* – visual confirmation of identity Hospital Identification number • Patient ID wristband or photo • Health record • Barcode
Verification of patient identity occurs: • At point of admission • Prior to providing any test, treatment, administering any medication or health service.
