Inspection (J) Inspection J Definition Visually and/or manually exploring a body part Explanation Visual exploration may be performed with or without optical instrumentation. Manual exploration may be performed directly or through intervening body layers Examples Diagnostic arthroscopy, exploratory laparotomy, diagnostic cystoscopy

Inspection (J) • The root operation Inspection represents procedures where the sole objective is

Inspection (J) • The root operation Inspection represents procedures where the sole objective is to examine a body part. • Procedures that are discontinued without any other root operation being performed are also coded to Inspection. • Procedures that are discontinued without any other root operation being performed are coded to Inspection.

Inspection (J) Coding Guideline B 3. 11 a. Inspection Procedures • Inspection of a body part(s) performed in order to achieve the objective of a procedure is not coded separately. • Example: • Fiberoptic bronchoscopy performed for irrigation of bronchus, only the irrigation procedure is coded.

Inspection (J) Coding Guideline B 3. 11 b. Inspection Procedures • If multiple tubular

Inspection (J) Coding Guideline B 3. 11 b. Inspection Procedures • If multiple tubular body parts are inspected, the most distal body part inspected is coded. • If multiple non-tubular body parts in a region are inspected, the body part that specifies the entire area inspected is coded. • Examples: • 1. Cystourethroscopy with inspection of bladder and ureters is coded to the Ureter body part value. • 2. Exploratory laparotomy with general inspection of abdominal contents is coded to the Peritoneal Cavity body part value.

Inspection (J) Coding Guideline B 3. 11 c. Inspection Procedures • When both an

Inspection (J) Coding Guideline B 3. 11 c. Inspection Procedures • When both an Inspection procedure and another procedure are performed on the same body part during the same episode, if the Inspection procedure is performed using a different approach than the other procedure, the Inspection procedure is coded separately. • Example: • Endoscopic inspection of the duodenum is coded separately when open Excision of the duodenum is performed during the same procedural episode.

Inspection (J) Coding Guideline B 4. 8 Upper and Lower Intestinal Tract Body Systems

Inspection (J) Coding Guideline B 4. 8 Upper and Lower Intestinal Tract Body Systems • In the gastrointestinal body system, the general body part values Upper Intestinal Tract and Lower Intestinal Tract are provided as an option for the root operations Change, Inspection, Removal and Revision. • Upper Intestinal Tract includes the portion of the gastrointestinal tract from the esophagus down to and including the duodenum, and Lower Intestinal Tract includes the portion of the gastrointestinal tract from the jejunum down to and including the rectum and anus.

Inspection (J) • Example: • In the root operation Change Table, change of a

Inspection (J) • Example: • In the root operation Change Table, change of a device in the jejunum is coded using the body part Lower Intestinal Tract. Coding Note: Procedures that are discontinued without any other root operation being performed are coded to Inspection.

Control (3) Control 3 Definition Stopping, or attempting to stop, postprocedural bleeding Explanation The site of the bleeding is coded as an anatomical region and not to a specific body part Examples Control of post-prostatectomy hemorrhage, control of posttonsillectomy hemorrhage

Control (3) • Coding Guideline B 3. 7 Control is used to represent procedures

Control (3) • Coding Guideline B 3. 7 Control is used to represent procedures performed to treat postprocedural bleeding. • If the following procedures are required to stop the bleeding, Control is not coded separately. Bypass Detachment Excision Extraction Reposition Replacement Resection

Repair (Q) • The root operation Repair represents a broad range of procedures for restoring the anatomic structure of a body part such as suture of lacerations. • Repair also functions as the Not Elsewhere Classified (NEC) root operation, to be used when the procedure performed does not meet the definition of one of the other root operations. • Fixation devices are included for procedures to repair the bones and joints.

Repair (Q) Coding Note: Limited NEC Code Options • ICD-9 -CM often designated codes

Repair (Q) Coding Note: Limited NEC Code Options • ICD-9 -CM often designated codes as Not Elsewhere Classified or Other Specified versions of a procedure throughout the code set. NEC options are also provided in ICD-10 -PCS, but only for specific, limited use. • In the Medical and Surgical section, two significant NEC options are the root operations value Q, Repair and the device value Y, Other Device. • The root operation Repair is a true NEC value. It is used only when the procedure performed is not one of the other root operations in the Medical and Surgical section.

Fusion (G) Fusion G Definition Joining together portions of an articular body part rendering

Fusion (G) Fusion G Definition Joining together portions of an articular body part rendering the articular body part immobile Explanation The body part is joined together by fixation device, bone graft, or other means Examples Spinal fusion, ankle arthrodesis

Fusion (G) • A limited range of procedures is represented in the root operation

Fusion (G) • A limited range of procedures is represented in the root operation Fusion, because fusion procedures are by definition only performed on the joints. • Qualifier values are used to specify whether a vertebral joint fusion uses an anterior or posterior approach, and whether the anterior or posterior column of the spine is fused.

Fusion (G) • Coding Note: • When coding fusions, it is important to understand

Fusion (G) • Coding Note: • When coding fusions, it is important to understand that PCS body part values are classified as Joints and not vertebra. • For example, if a fusion is done at L 1 -L 2, this is value 0, Lumbar vertebral joint (single), or one joint. If a fusion is done on L 1 -L 3, this is value 1, Lumbar Vertebral Joints, 2 or more, for 2 joints.

Alteration (0) Alteration 0 Definition Modifying the natural anatomic structure of a body part

Alteration (0) Alteration 0 Definition Modifying the natural anatomic structure of a body part without affecting the function of the body part Explanation Principal purpose is to improve appearance Examples Face lift, breast augmentation, cosmetic liposuction (liposuction for medical reasons is Extraction)

Alteration (0) • Coding Note: Alteration • Because some surgical procedures can be performed

Alteration (0) • Coding Note: Alteration • Because some surgical procedures can be performed for either medical or cosmetic purposes, coding for Alteration requires diagnostic confirmation that the surgery is in fact performed to improve appearance. • If the procedure is done for a medical condition, the appropriate root operation is assigned such as Extraction, Reposition, Resection, Repair, Replacement, and such.
