HSE LEARNINGS INCIDENTS: While driving on a graded road in Nimr C field, a pickup carrying four people on an unauthorised journey rolled over after turning at a “T“ junction at excessive speed. One passenger who was thrown out of the car died at the scene due to multiple fractures on his skull. Another passenger was found conscious but disoriented (LTI) and the remaining passenger and driver sustained superficial injuries (two medical treatment cases). ROOT CAUSES: A number of Life Saving Rules were violated: • Overspeeding. Failure to follow speed limits –he was driving at 90 km/hr instead of the 50 km /hr nighttime limit on a graded road • Failure of all the people to wear seat belts • Unauthorized journey –Night driving. Lateral Learning: • Night driving is not permitted unless authorised by approved authority • Wear your seat belt and ensure others do to. • Follow speed limits and road signs. • Reduce speed, STOP and then PROCEED at the Junctions • Check all rig vehicles comply with SP 2000 prior to any journey. • Ensure strict control on issuing vehicle keys Recommended Actions: Discuss this alert in toolbox talks and HSE meetings and Ø Distribute to all contractors Ø Post on HSE notice boards Ø Include in site HSE induction Key Learning: • Speed kills !
