HSE LEARNINGS Hazards when working close to heavy machinery Three fatalities due to hazards of working close to Heavy machinery in a period of less than 2 years in Our operations (Shell wide) 28 th April 2010 As a crane operator was retracting the outriggers and repositioning a crane, the crane reversed unexpectedly and crushed a welder against a 36” pipeline. The welder died from his injuries. 21 st May 2009 A Flagman trying to prevent a collision between a reversing grader and a car put himself in a position of danger. The grader driver did not hear/see any warnings, hitting the car. The Banks-man was run over by the grader and died from his injuries 28 th June 2008 A group of 6 workers was preparing a truck-mounted crane for relocation in a lay-down area on a construction site. During outrigger retraction, one of the workers reached across an extended outrigger beam. As the outrigger was retracted the worker was trapped between the chassis and the outrigger stabilizer cylinder. The worker died from his injuries Immediate action required: All CHs / CSRs at construction yards, logistics bases and other locations where people work in close proximity to heavy machinery should hold Tool Box Talks specifically to identify the hazards associated with this type of work at their location and to confirm that appropriate working practices are in place.
