Click NEW > FORM

Name the Form • Click “UNTITLED FORM” field • Name this form “Eating Habits”

Describe the Form • You can describe why you want your visitor to complete this survey • What ideas can you come up with for a survey on Eating Habits?

First Question • Google provides “Name” as the first question • With the tools on the right (for any question), you can EDIT, DUPLICATE, or DELETE the question

Create a Question • • Question Title – what are you asking? Help Text – any special instructions? Question Type – single, multiple, or Likert Scale Is this question Required to answer?

You can Email the Form You can type in email addresses, and all the recipients can click a link to take them to the survey.

You can Embed the Survey in Web 2. 0 Click MORE ACTIONS > EMBED, then COPY the link. Paste this into a wiki or blog site! We’ll do this later!
