HL 7 PUBLIC HEALTH WORKGROUP BIRTH DEFECTS IMPLEMENTATION
HL 7 PUBLIC HEALTH WORKGROUP BIRTH DEFECTS IMPLEMENTATION GUIDE BALLOT RECONCILIATION February 15, 2018
IG Structure Comments #29 (NEG) Commenter asked, “Why are there divergent ambulatory and inpatient documents? They share most of the same sections and this will mean duplicative work for EHRs and receiving systems. ” Included in the inpatient document are scenarios for inpatient–delivery admission, including stillbirth; inpatient–non delivery admission; and outpatient encounters. Collect data requirements from different jurisdictions that report to Public Health Agency. We cover business cases where a provider may have access to single episode-of-care or longitudinal care records for a patient, and this depends mostly on the type of care-setting. The differences on these data requirements are captured by different constraints in CDA header, section and entry levels in two document templates. An example of published IG that have different document level templates: HL 7 CDA® R 2 Implementation Guide: National Health Care Surveys Release 1, DSTU Release 1. 2 – US Realm August 2016 Disposition: Not Persuasive with Mod. We can put more narrative in the document templates to explain the different use cases (i. e. , This template describes constraints that are specific to the Hospital care-setting encounters which may include: Inpatient– delivery admission, including stillbirth; inpatient–non delivery admission; and outpatient encounters. ) 2
Recreational Drug Use Comments #66 (A-S) & #67 (A-S) Commenter noted that the "user" concept is a bit redundant with the drug, if the question is use. A single question "recreational drugs used" would provide the same information with less overhead. Commenter also noted that the History of Recreational Drug Use is more of an organizer than an observation, as it has no value other than those of its dependents. We agree with the commenter. Both “history of recreational drug use” and “recreational drug user” are used in 4. 26 History of Recreational Drug Use, but only one is needed. Recommend using “Type of recreational drug used during pregnancy” within 4. 26 to convey mother’s drug use. Disposition: Persuasive with Mod. § Delete the “Recreational Drug User” observation from 4. 26 History of Recreational Drug Use. § Simplify the observation to “Type of recreational drug used during pregnancy, ” which is contained in the mother’s social history section. 3
