How do you plan to use the Roadmap going forward and who will use it? Will a project manager or coordinator use it for project management? For reporting out? Will a leadership team/committee use it for benchmarking? For goal setting? Other ideas? Austin/ Travis County – United Way ATX 15 1. With who among your HMG team members and key partners have you shared the HMG Roadmap so far? - Key members on our 211, Connect. ATX & SX 6 team. We will be sharing it with other key partners in the future. 2. How do you plan to use the Roadmap going forward and who will use it? 1. The plan is for the project manager & the LT to use it for project management/setting action steps & keeping track of progress. 2. Educating stakeholders about steps & milestones 3. What activity(ies) in the Roadmap are already achieved or in progress already in your community? 1. We’ve secured some funding to bring a project manager on board 2. We already have a system working along side 2 -1 -1 that will act as our resource database (i. e. , Connect. ATX). 4. What activity(ies) seem at this time to be farthest down the road or most challenging for your community? 1. As a clear guide to the milestones we are currently working on and where we need to focus to achieve progress. Will a project manager or coordinator use it for project management? Yes, this will help focus our efforts. For reporting out? Yes to the larger coalition: Ready. Kid. SA Will a leadership team/committee use it for benchmarking? For goal setting? • Yes. This will be used for benchmarking progress and help establish our goals for the year. Committee met last week and was very appreciative of the tool. • We are scheduling monthly meetings to measure each milestone and establish our path. HELPMEGROWNATIONAL. ORG

Understanding the role of 211 and how it integrates into this process and does not duplicate while supporting families through an east access point 3. "Establish/customize an electronic early childhood resource directory through the collection and communication of information on community supports and services, to Centralized Access Point. Help Me Grow North Texas, MHMR Tarrant With who among your HMG team members and key partners have you shared the HMG Roadmap so far? 1. Our HMGNTX cross-sector Leadership Team 2. Our 5 HMGNTX Subcommittee Chairs How do you plan to use the Roadmap going forward and who will use it? 1. Leadership Team used it to create our overall HMGNTX Action Plan 2. Leadership Team created spreadsheet from Roadmap to breakout items pertaining to each model component/subcommittee for our local context. Family & Community Outreach gear became 2 subcommittees locally for Family Engagement and Marketing/Outreach. 3. Leadership Team used it to create our 5 Subcommittee Plans and gauged feedback and edits from Subcommittee Co-Chairs. Action Plans are living documents which co-chairs use to guide and track their work. 4. Action Plans were then used to create our HMGNTX Logic Model. 19 North Texas Area United Way, Wichita Falls, Texas With who among your HMG team members and key partners have you shared the HMG Roadmap so far? At this time only the project management team has viewed the roadmap. It will be shared with our Leadership team and key partners prior to being shared with our coalition members and other programs. How do you plan to use the Roadmap going forward and who will use it? We plan to use our in place data team to use the roadmap with our current data collecting/benchmarking, etc. The leadership team will work with various committees to connect the roadmap to our work to ensure we are meeting the needs of our community. What activity(ies) in the Roadmap are already achieved or in progress already in your community? Our key partner agreements are completed. The leadership committee is selected HMGTX will be introduced to our coalition during our November meeting. What activity(ies) seem at this time to be farthest down the road or most challenging for your community? While we have a great partnership with our Community Health Care Center and other health organizations, we expect Child Health Provider outreach to be a challenge for us! HELPMEGROWNATIONAL. ORG
